What do I need for my first appointment?

For your first appointment, you will need to bring

  • A valid state photo ID (non expired)
  • All of your insurance cards
  • Any x-rays/images reports or medical records that relate to your pain management
How early do I need to arrive for my appointment?

If you’re a new patient, please arrive 45 minutes before your appointment time to fill out forms. If you’re a follow up patient or have a procedure scheduled, please arrive 15 minutes before your appointment time.

What is a new patient consultation?
A new patient consultation is where you build a relationship with our providers to find the best pain treatment for you. You will have the opportunity to speak with our providers and they will go over your medical history. Please allow 45 minutes to 2 hours at the clinic so we can provide the best service.
How long will it take for me to get my authorization for my medication/procedure?
Medications can take up to 7 business days and procedures up to 14 business days. Due to new law, insurances will look into authorizations more carefully, which has extended the turn around time for prior authorizations. What can I expect if I’m taking opioids (narcotics) and my doctor will no longer write the prescriptions?

We will evaluate your list of medications during the initial visit and provide our recommendation about continuing opioid therapy. We may recommend another course of treatment that does not include opioids. We do not write prescription for opioids on the first clinic visit, so please do not arrive with the expectation that we will automatically refill your prescription.

How can I obtain my medical records?
Please download our medical records release form HERE and sign it and bring it into the clinic, or scan and email it info, or fax it to (206) 538-6301.
How should I prepare for my procedure?
Please alert us if you are on blood thinners, are pregnant or have any allergies. Depending on the procedure you might need to arrange for someone to drive you home. There are no eating or drinking restrictions with any of the procedures that we perform.
Why am I receiving a bill when I have multiple insurances?
Please present all of your insurance cards at the time of checkin, for all visits. This will avoid any confusion with the possibility of you receiving a bill in error and ensure we are billing accurately.
